Request Pet Records

Ensuring you have all your pets’ medical records is very important to us. Please fill out the form below to request those records or have them sent to your new veterinary hospital.

TIME FRAME: Please note that we will be checking these requests as often as possible, however it may take up to a week before they are emailed to you.

URGENT NEED: If you have an upcoming appointment at another veterinary hospital, please ask your new hospital to reach out to us via email instead of filling out this form. We do our best to respond to other clinics by the next day, often the same day.

CONFIDENTIALITY: We will only email records for your pet(s) if the email address provided matches the email address currently in our system. We cannot send records to an email address that does not match as this violates our confidentiality policy.

EMAIL ONLY: Unfortunately, we do not have any other method of providing you with past pet records. If you no longer have access to the email address in our system, please ask your new veterinary hospital to request the records and we will send the records to them directly.
